Getting your life back on track is not easy and it takes time to recover. It is important to see a GP or mental health worker to have a conversation and look at what is happening in your life. Our services

Country Wellness Connections (CWC)

CWC provides psychosocial support services to eligible participants in the Yorke and Northern Region of country SA.

While a diagnosis from a GP or other health professional is not required to access CWC, eligible participants must experience mental health concerns that are severe enough to impact everyday life. Participants must also not be assisted by the NDIS or be receiving other psychosocial supports that duplicates this service.

CWC is non-clinical, short term and flexible in its approach and is closely connected with mental health treatment services as per client needs.

 CWC services can include group-based and individual supports aiming to:

  • Assist Participants to identify their own individual needs and recovery goals
  • Build personal capacity and stability in areas as identified by the participant
  • Connect participants with mental health treatment services and other relevant support services as needed
  • Increase community participation and improve social connections to enhance mental health and general wellbeing of the participant

Participants with complex needs may also be supported to test their eligibility for NDIS.
